The Clinical Pharmacist Practitioner (CPP), Primary Care Service - Patient Aligned Care Team (PACT) e VANCHCS operates thirteen community-based outpatient clinics (CBOCs) covers ten county areas. These outpatient clinics are in Auburn, Chico, Fairfield, Mare Island, Martinez, McClellan, Modesto, Redding, Sacramento, Sonora, Stockton, Yreka, and Yuba City. The CPP works closely with other members of the health care team.Requirements:
Basic Requirements:- United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
- English Language Proficiency. Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
- Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia.
- Education. Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree.
GS-13 Clinical Pharmacist.
- Experience. One year of experience equivalent to the next lower grade level
- AND,
- Knowledge, Skills, and Abilities. In addition to the above experience, candidates must demonstrate they meet the following KSAs.
- Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
- Expert understanding of regulatory and quality standards for their program area.
- Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
- Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
- Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.
Preferred Experience: Candidates for the Martinez Clinical Pharmacist Practitioner (CPP) should possess the outlined skills with specific experience in the area of primary care chronic disease management. Candidates should have recent experience in a current or previous job (including residency training) that includes direct patient care and comprehensive medication management responsibilities in the Primary Care setting. Preferred candidates will have an advanced pharmacy degree (Pharm.D), board certification (BPS), PGY1 and/or PGY2 Pharmacy Practice Residency, or equivalent experience or mentorship.

Physical Requirements:
The incumbent requires adequate manual dexterity and fine motor coordination to utilize some of the instrumentation. The Clinical Pharmacy Specialist will move from a sitting/standing position several times during the day. Bending is occasionally required in this position. This position requires regular walking, sitting, stooping, standing, pushing, pulling and occasional moderate lifting of varying weights throughout the work week. It is not a sedentary position.
