Director, Chronic Condition Self-Management (Director ) CalOptima CalOptima Health is seeking a highly motivated an experienced Director, Chronic Condition Self-Management (Director ) to join our team. The Director for Chronic Condition Self-Management will lead the development, implementation and evaluation of all activities and personnel engaged in chronic condition self-management departmental operations. The incumbent will provide leadership, oversight and direction to the Chronic Condition
